We are pleased to inform you that all required athletic participation forms are online and must be electronically submitted before your son/daughter may be eligible to participate in Azle ISD athletics. Please follow the below steps to access and submit the forms provided:

1. Go to the following website: www.rankone.com.

2. Click on PARENTS CLICK HERE in the top right-hand corner.

3. This will take you to the Parent Portal, from there click ONLINE FORMS.

4. A screen will appear that asks you to select your state: Select TX.

5. The next screen will ask what the name of your school district is: Select AZLE ISD.

6. The site will then take you to the online forms page. Please ensure that you have read all instructions before beginning the forms.

7. After reading all instructions, click on the PROCEED TO ONLINE FORMS tab.

8. You will then create an account, or use your existing account to complete the online forms. Azle ISD does not have access to account usernames or passwords.

9. Go through each individual form and complete it in its entirety.

These forms may be completed online via computer, iPad, and/or cell phone. In the event you are unable to complete these forms at home please let your athlete’s coach know.
