Azle ISD provides campus RNs for nursing care. Additional nursing care provided for children who have complex medical needs is the financial responsibility of the parent or guardian. Please review the private duty nursing guidelines for more information.

Azle ISD Private Duty Nursing Requirements:

• Completed Request for Private Duty Nursing Authorization Form
• Completed District-Private Duty Nurse Agreement
• Completed District-Parent Private Duty Nurse Agreement

*Parents/guardians and private duty nurses must sign new agreements each school year. 
**Any nurse who will be providing care for a student on campus must complete the Azle ISD fingerprinting process prior to serving students on campus. 

Request for Private Duty Nurse Authorization:

Private Duty Nurse/Agency initiates the Request for Private Duty Nursing Authorization form.

Agencies and Private Dury Nurse:

• Read Azle ISD Private Duty Nursing Guidelines.
• Provide all necessary information required by Azle ISD.
• Receive, complete, and submit the Azle ISD District-Private Duty Nursing Agreement to the Director of Special Services.

Parents/Guardians:

• Read the Azle ISD Private Duty Nursing Guidelines.
• Provide all necessary medical documentation.
• Receive complete, and submit the Azle ISD District-Private Duty Nursing Agreement to the Director of Special Services.

Submit all required documents and forms to the Director of Special Services for review. Upon reviewing the documents, the administration will contact the parent/agency with approval.
