• All medications (prescription and non-prescription) must be brought to the clinic by a parent/guardian. Prescription medication must be in a current properly labelled pharmacy bottle. Non-prescription medications must be in the original bottle.   A maximum of a 30-day supply of medication will be accepted at a time.  Medications are not provided by the school. All medications must be picked up from the clinic by a parent/guardian at the end of the school year. 
    • A physician signature is required to administer over-the-counter medication for more than 10 school days.

     

    • A new medication form must be completed yearly by the parent and/or healthcare provider, depending on the type of medication ordered.

     

    • If the medication order changes at anytime during the school year, a new medication form will be required.

     

    • Out of state prescriptions and/or doctor orders will be accepted on a temporary basis with the parent/guardian establishing care with an in-state provider within 30 days of enrollment, per Texas Board of Nursing guidelines.

     

    • All medications will be kept in the clinic with this exception: a student may carry an inhaler, epi-pen, or diabetic supplies and medications by doctor's written order. Irresponsible behavior will revoke the privilege.

     

    • Students with diabetes must have a diabetic management plan updated yearly by the health care provider.  See diabetic guidelines for management of a student with diabetes for further information.

     

    • Any student with a special health need with conditions requiring special accommodations/medical management plans (e.g. diabetes, seizures, tracheostomies, cardiac anomalies, GTube feedings,  etc.) must have a plan/medical orders signed by a health care professional submitted to the school nurse annually.

     

    • Nonprescription, homeopathic medication, dietary supplements and herbals will only be given in accordance with Azle ISD board policies FFAC (legal) and FFAC (local). Aspirin or products containing aspirin will not be given without a physician's order.

     

    • The campus nurse is not responsible for medications or procedures for students who are not on campus.

     

    • Medications and procedures may be administered by non-medical staff who are authorized.

     

    • Medications that have expired or are not picked up by the parent or guardian at the end of the school year will be properly disposed.

     

    • Narcotic pain medication will not be administerd by school personnel.
