Only medications and procedures necessary to enable a student to remain in school will be administered.
Medications prescribed three times a day should be given at home in the morning, after school and at bedtime.
An Azle ISD medication form must be signed by both health care provider and parent/guardian and presented for all prescription medications or procedures.
Prescription medications must be in the current, properly labeled pharmacy container and brought to campus by an adult.
Only a three day supply of prescription pain relief medication will be stored on campus.
Over the counter medications may be given for 3 days with a signed note from the parent. To continue, will require the use of a medication form.
Medications sent unlabeled will not be given and will be destroyed.
All medications will be kept in the clinic with this exception: a student may carry an inhaler, epi-pen, or diabetic supplies and medications by doctor's written order. Irresponsible behavior will revoke the privilege.
Any change in the medication or procedure will require completion of a new form.
Herbal preparations, dietary supplements and other nutritional aids not FDA approved will not be given at school.
The campus nurse is not responsible for medications or procedures for students who are not on campus.
Medications and procedures may be administered by non-medical staff.
Medications must be picked up by the last day of school or they will be destroyed.
A newly completed form must be presented each school year.
