Azle High School Registration for  9th - 12th Grades

Tuesday, August 6th

7:30 - 11:30 am and 1:00 - 6:00 pm

All registration forms that are required are linked below!

Requirements for new students to enroll in Azle schools:

  1. Proof of Residency -- must be one of the following:
    1. Electric bill
    2. Water Bill
    3. Lease Agreement or House Contract
  2. Up-to-Date Immunization (Shot) Records
  3. Birth Certificate
  4. Copy of Student's Social Security Card
  5. Grades/Credits from Former School (High School)
    1. Withdrawal form from last school (for students transferring from another school during the school year.)
    2. Transcript (10th - 12th grade students)
    3. Last Report Card (9th grade students)
  6. Copy of Parent's Driver License
