Azle Elementary - Make Up Work Request Form

Parents and Guardians:

Please submit a "Makeup Work Request" form (below) if your child has been absent from school for three (3) full days or more.

Your child's homeroom teacher will collect the work from ALL of your child's teachers and have it available in the Office within 24 hours of receiving this form.

You may contact your child's teachers directly. Teachers' email addresses are available through the Directory.
